Terms & Conditions:
* For cataract surgery evaluations: Due to strict insurance regulation, if you have medical insurance plan that we they accept, the gift certificate cannot be used for your evaluation. Instead, your entire $1,200 certificate will be applied toward the elective portion of your cataract surgery, i.e., those not covered by insurance (laser astigmatism correction and Forever Young Lenses). If only last astigmatism is performed, the savings is $250 per eye (not $500 per eye).
** Discount and promotions are applied to full price services and procedures only. Surgical savings is $500 per eye for 3D Lasik and 3D FYL procedures, and $1,000 (one eye) for the 3D Kamra procedure. Discounts and gift certificates cannot be combined with any other certificates, promotions or discounts, and some restrictions apply.
